Course Overview
The RMTI Liver Transplant/Medicine Clinical Case Review Conferences (RMTI Liver CCRC) bring together clinicians from hepatology, hepatobiliary surgery, surgical oncology, nursing, anesthesiology, critical care, radiology, medical subspecialties, virology, genetics, pathology, transplant immunology, infectious diseases, and psychiatrists, as well as social workers, nurse and financial coordinators and pharmacists. The ability to transplant patients having HCC is complex and involves multiple disciplines. The need to pursue deceased donation versus live donation, what therapeutic regimen to use and appropriate follow-up of patients is complex and critical to discuss in a generalized forum in a multidisciplinary manner. Organ allocation policies and national listing priorities change often and affect the working of patients and hence the protocols that physicians and coordinators follow. The conferences provide a HIPAA secure environment to discuss the needs of each patient in a multidisciplinary forum and develop an individualized treatment plan, in addition to identifying any education gaps and implementing quality improvement/corrective action plans. RMTI Liver CCRC fosters and promotes the ACGME Core Competencies Patient Care, Medical Knowledge, Interpersonal and Communication Skills, Professionalism, Practice-Based Learning and Improvement, and Systems-Based Practice.

Objectives
  1. Asses patients who can undergo medical therapies for treatment of alcohol hepatitis versus those with more severe disease who will need listing for early liver transplantation.
  2. Assess patients who were transplanted for alcohol hepatitis to monitor for any tendency of recidivism as they will require intensive follow-up to avoid the risk of liver rejection.
  3. Explain to patients that they must reach out to their hepatologist and transplant coordinator if they are having thoughts of relapse back to drinking alcohol. Patients should enlist the help of the multi-disciplinary transplant team, such as social work, psychiatrists as well as other social supports to mitigate the tendency of recidivism post-liver transplant leading to potential loss of life.
