Course Overview

Please join us on April 14th, 2021, from 6:00-7:30 pm for a live virtual CME event: Frontline Excellence in Ambulatory Chronic Disease Management, Module 2: Hypertension and Chronic Kidney Disease. Dr. Roy Cohen, Dr. Joji Tokita, and Dr. Steve Coca will share the latest evidence-based practices for managing hypertension and chronic kidney disease in the outpatient setting.

We invite all Mount Sinai Health System employees and community-based primary care physicians, nephrologists, nurses, physician assistants, and nurse practitioners to join us for an insightful and engaging evening of learning. 

The goal of this CME activity is to provide physicians and their clinical staff education on how to successfully manage patients with hypertension and patients with chronic kidney disease in the ambulatory setting, following best practices for classification, medication management, and multi-disciplinary collaboration.


Objectives

At the conclusion of the event, physician participants will earn a maximum of 1.5 AMA PRA Category 1 Credits™ and be able to:


  1. Outline lack of optimal screening and treatment plan for earlier stage chronic kidney disease
  2. Understand ambulatory management, medication considerations, and referral goals for Chronic Kidney Disease (CKD) & Essential Hypertension (HTN)
  3. Understand multidisciplinary team members and roles to optimize disease management for CKD & HTN
  4. Understand the scientific underpinnings of new screening options for better risk stratifying stage 3 CKD
