Course Overview
The goal of this CME activity is to provide physicians and their clinical staff education on how to successfully manage chronic disease in the ambulatory setting, following best practices for classification, medication management, and multi-disciplinary collaboration.

Objectives
  1. Outline optimal ambulatory management and medication considerations for Diabetes Mellitus (DM) type II and Heart Failure (HF)
  2. Recognize value of multidisciplinary teamwork to optimize disease management for Diabetes Mellitus (DM) type II and Heart Failure (HF)
  3. Understand management consideration for HF sub-types (HFREF, HFPEF, RHF).
  4. Outline appropriate usage and considerations for diabetic medications including SGLT2 inhibitors and DPP-4 inhibitors
