Course Overview
The Healthspan conference aims to educate an interdepartmental audience on best practices of Age Friendly Care. Subject matter experts will present on the various components of health span and how to appropriately provide Age Friendly Care.

ACCME Objectives
  1. Describe and apply the principles of the Age-Friendly Health Systems 4Ms Framework (What Matters, Medication, Mentation, and Mobility) to develop individualized care plans that align with evidence-based age-friendly care practices for older adults.
  2. Incorporate validated healthspan measures, including functional, cognitive, mobility, and quality-of-life assessments, into the evaluation and management of older adults to support healthy aging and preservation of independence.
  3. Identify and implement age-friendly care strategies that reduce preventable complications, improve transitions of care, and decrease avoidable healthcare utilization among older adults across clinical settings.

ANCC Learning Outcomes
  1. Describe and apply the principles of the Age- Friendly Health Systems 4Ms Framework (What Matters, Medication, Mentation, and Mobility) to develop individualized care plans that align with evidence-based age-friendly care practices for older adults. (Knowledge)

The Icahn School of Medicine at Mount Sinai is accredited by the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing medical education for physicians.
