Course Overview
The goal of this CME activity is to provide physicians and their clinical staff education on how to successfully manage patients with polychronic disease in the ambulatory setting, following best practices for screening and classification, treatment, and multi-disciplinary coordination and collaboration.

Objectives
  1. Discuss the leading clinical screening and classification, appropriate treatment, and follow-up and coordination considerations for patients with complex polychronic disease.
  2. Describe the different assessment steps to identify a polychronic disease state patient
  3. Distinguish the appropriate point in disease progression/ complication to refer to specialist
  4. Apply appropriate disease management techniques for patients with two or more chronic conditions
  5. Identify and recognize the appropriate time to refer patient to other resources (including care management and pharmacy support) to improve patient outcomes and patient satisfaction

Accreditation
The Icahn School of Medicine at Mount Sinai is accredited by the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing medical education for physicians.
